To sponsor one or more Legacy Plaques, please fill out and submit the form below. You will have the option of paying by check or credit card.

HANAC LEGACY PLAQUES

$1,000 - Legacy Apartment Plaque

Each plaque will be placed next to an apartment door in the residence of your choice. Decorative bronze 2-inch by 6-inch oval plaque. Personalized with one to four lines of text.

First choose the residence of your choice. You will then see an area in which to provide us with the text for that plaque. Our staff will contact you to finalize your text before ordering your plaque.

You may add as many Legacy Apartment Plaques as you choose, for a $1,000 donation each.

$500 - Legacy Bench Plaque

Each plaque will be placed on one of the benches in the patio or terrace areas of the residence of your choice. Decorative brass 2-inch by 6-inch plaque, brushed and laquered for protection against the elements.

You may add as many Legacy Bench Plaques as you choose, for a $500 donation each.
